ENROLLMENT PROCESS FOR SERVICE DOG PROGRAM

PRE-ENROLLMENT STEPS

  1. Complete an initial consultation and answer questions from the client. The client communicates the desire to proceed with application to the program. 

  2. The client completes and submits Traits Inventory (Human) - Service Partner. 

  3. The client completes and submits Traits Inventory (Canine) - Service Dog in-Training Candidate.

  4.  Client and trainer decide on the Service Dog In-Training. 

  5. The client and his/her/their family complete Family Inventory

  6. The client completes and submits Health Form for the Service Dog-in-Training chosen.  

ENROLLMENT STEPS

  1. Review the materials submitted above and decide if you want to enroll the student. Consult with PHD as appropriate so that you feel confident of your decision and can anticipate and plan for challenges that could arise in working with this client/dog. 

  2.  Email the client that their application for enrollment in the Service Dog Training Program has been reviewed and will be approved upon your receipt of the client's signed Training Agreement - Service Dog

POST-ENROLLMENT STEPS

  1. Email the client telling them that they are now officially enrolled in the Service Dog Training Program. 

  2. Email the student a signed copy of the Letter - Service Dog in-Training

  3. Schedule your first class(s): start with the in-home training section of the curriculum.
