1. NEW CLIENT FORMS

New clients need to submit all three forms below before booking requests will be reviewed or accepted. 

  1. Agreement for Services  

  2. New Client Questionnaire  

  3. Bite History. Complete the form even if your dog has no bite history (follow the instructions on the form). To complete the form for a dog WITH bite history, use this bite scale to rate the level of each bite. 

2. VOLUNTEER FORM

If anyone in addition to the client is to be present at session, they must sign a volunteer form before session begins. 

3. EXISTING CLIENT FORMS

  1. Incident Report. This form should be completed within 24 hours if any of the following conditions have occurred during practice: a) client encounters an access challenge to an environment, or b) client’s dog exhibits an unsafe or inappropriate behavior (for example, lunging at, barking at, biting, growling at, snarling at, or snapping at another dog or human).

  2. Request to Change Monthly Tuition Hours. This form must be completed before 10 days of the new month. 

  3. Registration Renewal for Service Dog  This form is to be completed yearly by Service Dog Program graduates. Upon receipt of the form, PHD will bill the client for the renewal fee + ID fee. These are identified in the PHD catalog
